Similar fix, but has separate code paths for "here's a new vendor, allocate a
new entry and add it to both hash tables" and "here's new information for an
existing vendor with a given ID, update the information and, *if and only if*
the name changed, remove it from the by-name table, free the name, update the
name, and add it to the by-name table with the new name".
