A few fixes and more cleanup
Fixes a few typos/bugs that slipped in during the patching and removal of 2 out
of 3 globals. I removed a lot of the duplicate macros and special code that
was associated with those globals. I changed a bit of formatting/naming to
keep the code consistent throughout the file.
