Patch to add extension support v5

v5 of the patch has the following changes from v4:
 - reports malformed GLX/Render subrequest sizes via expert_add_info_format
instead of REPORT_DISSECTOR_BUG
 - uses custom structures instead of abusing value_string
 - indents generated code for 'struct' functions properly.
 - implements xcb <union> support, prompted by:
 - omits unused structure dissectors via the brute-force method of blacklisting
the ones that are never used.
