I know that   http://www.networkchemistry.com/products/packetyzer.php  can send one packet at a time.  I don't know about replaying the whole thing.

http://www.karalon.com/ may or may not have a free version.  

Spirent's Avalanche can do what you want and more (although the UI is a bit quirky).  Of course, Avalanche is not free.

To change the ip/mac addresses, search for "pcap anonymizer" and you'll get plenty of hits.
