I think that Packetyzer www.packetyzer.com (Windows GUI + ethereal based lib)  has a feature to edit and replay packets.
Maybe it hasn't all the features you want.
 
If you are using e.g. Linux you could maybe look at tcpreplay.
There is also a program called netdude that can be used as a packet editor for libpcap files.
