I think it is related to the reassemble of t30. Please provide the file to take a look. Also notate that the reasemble only works for the first two Items in a t38 packet, so if there are more than two items, that can lead to a miss reassemble packet.

ITU-T Recommendation T.30? :-) That'd either give you information to add them, or tell you that they're non-standard.

Or is this a case of reassembly not being done?

Do you have a sample capture with this problem?
